Basic Life and AD&D
Summary of Benefit
To ensure that all eligible employees have a basic level of protection, the City of Chattanooga provides Basic Life and AD&D Insurance at no cost to you. Basic Life and AD&D provides financial security to you and your family in the event of an unforeseen hardship from your death or dismemberment.
This is a group term life and accidental death and dismemberment insurance policy that is provided to all full-time employees. This policy is convertible to an individual policy upon termination.
Eligibility
Full-time employees are eligible for coverage the first day of the month following 30 days of continuous active employment.
Amount of Coverage
Eligible employees receive an amount equal to their stated annual salary rounded to the next $1,000, but no more than $50,000.

Long-Term Disability
Summary of Benefit
Long-Term Disability provides financial benefit in the event of a total and permanent disability. This plan is designed to provide you with an income of approximately 60% of the amount of your earnings prior to becoming disabled.
Eligibility
Full-time employees who participate in the General Pension Plan are eligible are eligible for coverage after 180 days of employment. The City pays for this coverage for all eligible employees.
Amount of Coverage
The financial benefit is a monthly income of 60% of your monthly rate of pay prior to becoming disabled. Benefits from the LTD plan are adjusted based on income you may receive from other disability plans. The amount of the benefit you may receive will not be less than $100 per month or more than $7,500 per month.
Maximum Benefit Period
Disability benefit payments are paid after a 180 days elimination period. Please refer to the Summary Plan Description (SPD) for the schedule of the maximum duration of benefits.
